unified patch

I collected whatever patches I could find and merged them into one patch
for
personal usage, perhaps someone else will find it useful as well..

this patch is applied against ./rdesktop-patched and the source should
be
rdesktop-1.0.0.

included patches are:

        the 16-bit Xserver patch support - patch level 7
        proper cursor support patch
        small upside-down images patch
        txt handling patch
        w2k patches -- caplen +=4, order_caps fix and the FillOrMix
patch.

        did I miss something? Did I get them right? ;)

I dunno how well this will work with nt 4 servers since we don't have
anyone
installed anymore, however it kinda works with a w2k machine.
